DOES MAMMOTH PHYSICAL THERAPY ACCEPT INSURANCE?

Mammoth Physical Therapy is an out-of-network provider and does not accept insurance directly. However, we can provide you with invoices and detailed treatment plans that you can submit to your insurance company for reimbursement.

WHAT QUESTIONS SHOULD I ASK MY INSURANCE COMPANY ABOUT OUT-OF-NETWORK SERVICES?

When speaking with your insurance provider, it's important to ask the following questions:

• Does my plan provide out-of-network benefits?

• What percentage of the cost will be covered?

Is there a deductible or coinsurance for out-of-network providers?

• Is there an out-of-pocket limit for out-of-network providers?

• What documentation do I need to submit for reimbursement?

Do I need a doctor’s referral?

California has a Direct Access law that allows patients to access physical therapy services without a physician's referral. This allows patients to receive physical therapy treatment directly from a licensed physical therapist without having to contact a physician first. The California Direct Access Law for Physical Therapy applies to all physical therapists licensed in the state, and enables physical therapists to evaluate, diagnose, and treat patients without first obtaining a referral from another healthcare provider. This law provides greater access to physical therapy services and allows patients to receive the care they need in a timely manner.

What is your new patient procedure?

At Mammoth Physical Therapy clinic, we require an Initial Evaluation service to establish care at our facility. A physical therapist will conduct a thorough evaluation and develop a personalized plan of care, which may include Manual Therapy, Therapeutic Exercise, and Neuromuscular Re-education. We use a battery of tests and measures in order to create a snapshot of your overall physical health, including force plate testing, Manual Muscle Testing, Range of Motion testing and orthopedic special tests.  You can expect to be issued a Home Exercise Program at this visit in order for you to start your management of your issue immediately.

WHAT SHOULD I WEAR AND BRING TO MY APPOINTMENT?

It is best to wear comfortable clothing that allows you to move freely. Avoid wearing restrictive clothing such as jeans or tight-fitting clothing. It is also important to wear shoes that provide good support and cushioning.  Please bring a filled water bottle and any medications you are currently taking. It is also beneficial to bring any imaging studies such as an X-ray, MRI, or CT scan that your doctor has ordered.
